Required Training and Acknowledgement Forms for 377 Staff
Must be completed by September 12, 2022
The annual required trainings will all be conducted online from Greenbush. All Trainings should be watched in their entirety and intently. Please take the time to learn what the videos are presenting and ask your supervisor if you have any questions regarding the content.

Once all video trainings have been completed. YOU MUST Fill out this Acknowledgement Form: DO NOT SUBMIT ON THE LINK ON THE WEBSITE.
The videos will state who needs to do them and how often. STAFF DO NOT HAVE TO DO STRUCTURED LITERACY AND DYSLEXIA TRAINING.
Acceptable Use Policy Information – all district staff members who use of district technology, network, and internet resources are required to be knowledgeable of the district Acceptable Use policy and must sign an Acceptable Use Policy Acknowledgement form
Asbestos Notice – all USD 377 staff are required to submit an Asbestos Notice Acknowledgement form.
Kansas Tort Claims Act for Teachers – this form must be submitted annually by all teachers and counselors.
Hepatitis B Information – Hepatitis B information is taken from the USD #377 Safety Handbook. The Hepatitis B Informational Packet/Study Guide below is for new employees who are required to file a Hepatitis B Consent or Declination form.
Safety Handbook Informaion – the Safety Handbook states the employer's policies and practices in effect on the date of publication. To the extent that the USD #377 Employee Safety Handbook sets out policies, that are to be complied with by all employees. Failure to do so is a violation and may result in disciplinary action up to and including Termination. All employees are required to submit a Safety Handbook Acknowledgement Form.
Transportation Disclosure Form – the form is for any school employee who transports students.
Staff information and leave forms – Update your contact information and sign up for the sick leave bank if interested.
PDP Toolbox Training – for certified staff who file for professional development points. PDP training is optional and provided to assist certified staff with PDP account setup and use.
