Out-of-District School Attendance and Transportation application available for 2017-18 / -This form must be signed and resubmitted every year-

K.S.A. 72-1046b allows a parent that resides 2.5 or more miles from the attendance center their child(ren) would attend to apply for attendance and transportation at another district.

1. This application is given to the board of the receiving school district, requesting to be furnished transportation as a non-resident pupil.

2. If the receiving board agrees to furnish transportation, it will be provided until the end of the school year.

3. Prior to providing transportation, the receiving district should provide a copy of the application to the school district of residence.

4. This application must be resubmitted every year and must be signed by the parent or legal guardian.

The application form for school year 2017-2018 is now available to use at kindergarten roundup, and to have parents renew their application for the upcoming school year. NOTE: This statute excludes school districts with territory in Johnson, Sedgwick, Shawnee, or Wyandotte counties.

YOU MUST USE YOUR PARENT LOGIN INFORMATION THAT CAN BE OBTAINED FROM THE SCHOOL IF YOU HAVE NOT YET SET UP YOUR PARENT PORTAL.

To the Parents/Guardians

We are proud to announce that you can now enroll your student online and have access to update your student's information 24/7. If you have internet access you can view the information on your student at any time. All you need to do is create a Parent Account.

This allows for single sign-in, if you have more than one student enrolled in Atchison County Community Schools you will now be able to access all of your students with one account. You will need the Access ID & Password for each student you would like set up on the account using the instructions below.

How to Create a Parent Account
1. Open your Web browser http://powerschool.usd377.org/public. The Sign In page appears. BECAUSE YOU DO NOT HAVE A PARENT ACCOUNT, BE SURE YOU CLICK “CREATE ACCOUNT”

2. Click the Create Account tab.


 
3. Click Create Account button. 


 

4. Use the following table to enter information in the Create Parent Account section:

First Name - Enter your first name.

Last Name - Enter your last name.

Email - Enter your email address. The email address you enter is used to send you select information, as well as account recovery notices and account changes confirmations. 

Desired Username - Enter the username you would like to use when signing in to the PowerSchool Student and Parent portal. The user name must be unique. If you enter a user name that is already in use, you will be
prompted to select or enter another user name.

Password - Enter the password you would like to use when signing in to the PowerSchool Student and Parent
portal. 

Re-Enter Password - Enter your password again exactly as you entered it in the above

5. Use the following information will help you Link your student’s account to your account.:

Student Name - Enter the first and last name of the student you want to add to you account. Note: Regardless of the name you enter, the system will populate the name based on the access ID and password for the student.

Access ID - Enter the unique access ID for the student.

Access Password - Enter the unique access password for the student.

Relationship - Indicate how you are related to the student by choosing the appropriate association from the
pop-up menu.

6. Be sure you enter access ID, password, etc., for each child you will be enrolling. YOU ONLY GET ONE ACCOUNT, THEREFORE IF YOU DO NOT LINK ALL YOUR CHILDREN YOU MUST DO SO LATER FROM THIS ACCOUNT.

ONCE YOU HAVE ENTERED ALL OF YOUR CHILDREN, SCROLL TO THE BOTTOM AND CLICK ON “SUBMIT”

Please keep your passwords confidential so only you can access the information.

If you have difficutly creating your account, we recommend that you attend the Enrollment Walk-in session or call the school where your child attends and someone will be able to assist you.

    This is your application used to determine if you qualify for Reduced Price or Free School meals. Print and complete this form and bring to enrollment with you. Per KSDE, this form can only be submitted in paper format - online is not allowed.

    To save you time and effort, information about your children’s eligibility for reduced price or free Child Nutrition Program benefits may be shared with other programs for which your children may qualify.  For the programs listed below, we must have your permission to share your information.

KANSAS STATE HIGH SCHOOL ACTIVITIES ASSOCIATION
RECOMMENDATIONS FOR COMPLIANCE WITH THE KANSAS SCHOOL SPORTS HEAD INJURY PREVENTION ACT AND IMPLEMENTATION OF THE NATIONAL FEDERATION SPORTS PLAYING RULES RELATED TO CONCUSSIONS

The following language appears in all National Federation sports’ rules books:
“Any athlete who exhibits signs, symptoms, or behaviors consistent with a concussion (such as loss of consciousness, headache, dizziness, confusion, or balance problems) shall be immediately removed from the contest and shall not return to play until cleared by an appropriate health care professional.”

The Kansas Legislature has enacted the School Sports Head Injury Prevention Act (hereinafter the “Kansas Act”) effective July 1, 2011:

Sec. 72-135. (a) This section shall be known and may be cited as the school sports head injury prevention act.
(b) As used in this section:
(1) ‘‘School’’ means any public or accredited private high school, middle school or junior high school.
(2) ‘‘Health care provider’’ means a person licensed by the state board of healing arts to practice medicine and surgery.

(c) The state board of education, in cooperation with the Kansas state high school activities association, shall compile information on the nature and risk of concussion and head injury including the dangers and risks associated with the continuation of playing or practicing after a person suffers a concussion or head injury. Such information shall be provided to school districts for distribution to coaches, school athletes and the parents or guardians of school athletes.

(d) A school athlete may not participate in any sport competition or practice session unless such athlete and the athlete’s parent or guardian have signed, and returned to the school, a concussion and head injury information release form. A release form shall be signed and returned each school year that a student athlete participates in sport competitions or practice sessions.

(e) If a school athlete suffers, or is suspected of having suffered, concussion or head injury during a sport competition or practice session, such school athlete immediately shall be removed from the sport competition or practice session.

(f) Any school athlete who has been removed from a sport competition or practice session shall not return to competition or practice until the athlete is evaluated by a health care provider and the health care provider provides such athlete a written clearance to return to play or practice. If the healthcare provider who provides the clearance to return to play or practice is not an employee of the school district, such health care provider shall not be liable for civil damages resulting from any act or omission in the rendering of such care, other than acts or omissions constituting gross negligence or willful or wanton misconduct.

(g) This section shall take effect on and after July 1, 2011.

This record is part of the student's permanent record and shall be transferred from one school to another as defined in Section 72-5209(d) of the Kansas School Immunization Law (amended 1994.) KCI MAY ONLY BE SIGNED BY A PHYSICIAN (MD/DO), HEALTH DEPT, OR SCHOOL.

Legal Alternatives to Vaccination Requirement "KSA 72-5209"

  • "Annual written statement signed by a licensed physician (Medical Dortor/M.D. or Doctor of Osteopathy/D.O.) stating the physical condition of the child to be such that the tests or inoculations would seriously endager the life or health of the child." Medical Exemption shall be validated annually by physician completion of KCI Form B and attachment to the KCI.
  • "Written statement signed by one parent or guardian that the child is an adherent of a religious denomination whose religious teachings are opposed to such tests or inoculations.

IMMUNIZATION INFORMATION


The Kansas Certificate of Immunization record is part of the student's permanent record and shall be transferred from one school to another as defined in Section 72-5209(d) of the Kansas School Immunization Law (amended 1994).

Kansas immunization requirements are based on age of child as of September 1 of current school year. As per Kansas Statute 72-5209, all children upon entry to school must be appropriately vaccinated.

Prior to any refund being approved, funds will be applied to any existing balances. The district may turn over delinquent accounts to the School Board Attorney’s office for collection.
