UPDATING CURRENT INFORMATION - Student Demographic Information should be updated annually or anytime important parent/guardian contact information changes. In order to update student contact information, such as address change, emergency contact changes, phone number changes or any other changes, please use the form below:
NEW TO OUR DISTRICT?

OUT OF DISTRICT STUDENTS
Out of district students must be approved prior to being allowed to enroll.

Enrollment guidelines for Pre-school, Kindergarten and All Students New to our District:
Preschool, kindergarten and new to the district first-graders:
- A certified state copy of your child's birth certificate
- A copy of your child's immunization records or written validation of medical or religious exemption (list of state required immunizations)
- A copy of your child's Social Security number
- Preschoolers and kindergartners are required to have a physical exam in accordance with Kansas Statute 72-5214
- By Kansas law, children must be five years old on or before Aug. 31 to enter kindergarten
Parents may apply for a birth certificate for their child by visiting the Kansas Department of Health and Environment Web page and downloading the appropriate form. (Visit the department's site at http://www.kdheks.gov/vital/birth.html for a link to PDF forms.)
New to the district students entering second- through fifth-grade:
- Proof of identity (one of the following)
- Certified state copy of your child's birth certificate
- Certified transcript of school records
- Actual school records from the sending school
- Hospital certificate of birth
- Immunization records or written validation of medical or religious exemption (list of state required immunizations)
- It is useful to bring a copy of your child's most recent report card, and your child's Individual Education Program (IEP), if your child has one.
- It is also helpful to bring proof of your legal guardianship if your last name is different from the child's last name.
- A physical is recommended for all new students entering the district.
New to district students entering sixth- through 12th-grade:
- Proof of identity (one of the following)

- The annual physical must be obtained after May 1 for use in the following school year.
- All items on the form must be completed.
- A parent's signature is required for student participation.
- A new physical is required each school year for sports participation.
